SUMMARY:

Serving more than 200,000 patients annually, California Pacific Medical Center (located in beautiful San Francisco) provides a wide variety of services including: acute hospital care, a full range of post-acute care, home care and hospice services, outpatient medical care, and preventive, complementary, and educational services. The Medical Center also provides professional education and basic biomedical, clinical and behavioral research. PRIMARY PURPOSE:

The Fetal Treatment Nurse Navigator serves as the primary link between the medical physician, the maternal/fetal patient, and the services and clinicians who are coordinated in the context of a comprehensive treatment plan.
The Fetal Treatment Nurse Navigator provides care coordination services for every patient who enters treatment in the Sutter Perinatal & Neonatal department. This role will include, but is not limited to:
• Educate on disease specifics and treatment options.
• Assessment and planning for comprehensive surgical and subspecialty service coordination.
• Improve coordination of treatment and support.
• Provide a link between the patient and the physician.
• Facilitates access to support networks and psychosocial assistance.
• Enable informed follow-up care
• Oversees clinical staff working in the medical office setting to enable them to support the plan of care.
• Manages complex computer program to develop treatment plans in partnership with the Perinatal/Genetics and Neonatal Physicians.
• Ensures that a electronic health record and tracking is used to optimize patient safety and communication.

Qualifications
Education:
Bachelor's Degree in Nursing Required
Masters Preferred

Qualification:
Registered Nurse - State Licensure - RN required

Minimum of 3 years of Neonatal/OB Nursing experience required working in NICU or OB units; RN navigator or coordinator experience preferred.

Knowledge:
Computer proficiency in Microsoft Office, including: Excel, Access, Word, PowerPoint ; familiarity with Customer Relationship Manage Software platforms; Familiarity with Telemedicine platforms
Demonstrated ability to effectively and efficiently manage own time with a minimal level of supervision.
Ability to read, write, hear, and communicate verbally in English to the degree required to perform the job.
Excellent verbal and written communication skills required and ability to interact assertively and professionally with members of the medical staff and with a wide variety of personalities and departments.
Willingness to dress and act in a professional manner at all times.
